1st and 2nd Draw PPP Loan Applications for Self-Employed Individuals filing a Schedule C:
The online portal will require you to enter an email address of which you will have to validate through a direct/instantaneous email back to your account (if you don’t immediately see this email, please check your spam account). Once you validate your request, please follow the steps within the portal which will include uploading the requested documents above. Once submitted, a Harvest representative will reach out directly with any additional items and/or security questions to validate you as a borrower. Once those items have taken place, the Harvest representative will submit to the SBA for approval. Be cautioned this could take a few days to clear the SBA center due to high demand. After an approval is in hand, you will be notified of your approval with a copy of your loan documents. You will need to return those executed documents along with an ACH form for the proceeds to be wired. We expect that to occur within 48 hours of receipt.

- Last 4 quarters 941 payroll tax forms
- Payroll roster or log to verify employees & salaries
- Driver’s License
2nd Draw PPP Loan UNDER $150,000 Request (Existing PPP Harvest 1st Round Borrower):

- Documentation to support a 25% reduction in Annual 2020 vs. 2019 gross revenues OR a 2020 vs 2019 comparison using the 1st, 2nd, 3rd, or 4th quarterly figures from the same period the previous year
- Documentation may include relevant tax forms, including annual tax forms (2020 & 2019), or, if relevant tax forms are not available, quarterly financial statements or bank statements
2nd Draw PPP Loans - NON Harvest 1st Round Borrower:
- Copy of NOTE from 1st PPP Loan
- Supporting Payroll documentation used in 1st PPP Loan
